AFA HUB provides individualized Self-Directed Support Services that empower participants to design, manage, and direct supports aligned with their personal goals, interests, and strengths. Our services are rooted in person-centered planning and are designed to support individuals participating in Self-Determination Programs by offering flexible, goal-driven options that promote independence, skill development, and meaningful community engagement.
Through a collaborative and participant-led approach, AFA HUB supports individuals and families in navigating self-direction while ensuring services are clearly defined, outcome-focused, and compliant with program guidelines.
Self-Directed Support Services May Include:
