AFA HUB provides individualized Self-Directed Support Services that empower participants to design, manage, and direct supports aligned with their personal goals, interests, and strengths. Our services are rooted in person-centered planning and are designed to support individuals participating in Self-Determination Programs by offering flexible, goal-driven options that promote independence, skill development, and meaningful community engagement.

Through a collaborative and participant-led approach, AFA HUB supports individuals and families in navigating self-direction while ensuring services are clearly defined, outcome-focused, and compliant with program guidelines.

Self-Directed Support Services May Include:

• Person-centered planning support to identify goals, interests, strengths, and desired outcomes

• Assistance implementing participant-directed services outlined in individualized plans

• Career exploration and employment readiness activities

• Creative and technical skills training in areas such as film, animation, video game design, visual effects, voice-over, and emerging AI tools

• Project-based learning and real-world skill application

• Micro-enterprise and self-employment exploration and support

• Community-based learning and engagement opportunities

• Support with scheduling, coordination, and organization of services

• Progress tracking, documentation, and outcome reporting

• Collaboration with families, Independent Facilitators, Financial Management Services (FMS), and service coordinators
